JOB SUMMARY

Whilst functioning as an integral member of our primary care team, the Registered Psychiatric Nurse not only meets the immediate needs of individuals during moments of crises, but also bridges care across our various health teams including home and community care, public health, and the mental wellness team. In caring for clients longitudinally over time, this role also provides essential support and care planning before and after client admissions to treatment centers and acute care hospitals to ensure client safety and continuity of care across such transition points.

 

DUTIES & RESPONSIBILITIES

  • Works collaboratively within an interdisciplinary team-based model of care to provide client and family-centered nursing care
  • Using a trauma-informed approach, promotes a culturally-safe and therapeutic care environment in support of client self-advocacy and self-determination
  • Supports an integrated and holistic approach to care planning which addresses all aspects of health and wellbeing, including the spiritual, the emotional, the physical, and the mental
  • Supports program delivery and community engagement across all health teams

 

Client Care Management:

  • With cultural humility, fosters care relationships with clients and their self-identified families and support persons to help establish rapport, trust, and mutual respect
  • Assesses clients’ functional status and informal support network, and bolsters integrity of supports as needed by way of advocacy, shared care planning, and integration of additional formal supports and resources
  • Anticipates medical and functional changes over time, especially in the context of serious illness or chronic disease, and provides avenues for early engagement and conversations with clients and families to ensure care planning is guided by clients’ and families’ goals and values
  • Initiates and participates in care conferences with clients, families, inter-professional team members, and service providers to help ensure a coordinated and integrated approach to client care
  • Supports clients’ access to health services and helps clients and families navigate resources and overcome personal and systemic barriers to accessing care and achieving health & wellness

 

Nursing Care & Counselling:

  • In conducting health assessments, helps identify clients’ physiological, psychological, sociocultural, and spiritual needs to ensure a holistic approach to care planning and care provision
  • Coming from a client & family-centered model of care planning, with clients as partners in their care, tailors individualized care plans in consultation with clients, their families, and members of the interdisciplinary care team
  • Provides nursing treatment services including crisis intervention, individual counselling, and health education, whilst also facilitating group education and therapy sessions
  • Delivers trauma-informed opioid agonist treatment (OAT) health care services, including physical, psychiatric, and substance use assessment; administering prescribed treatments and medications; and monitoring withdrawal symptoms and evaluating response to treatment
  • Liaises with community Elders and Knowledge Keepers to help foster connection, cultural engagement, and strength-based healing

 

QUALIFICATIONS

  • Current practicing registration as a Registered Psychiatric Nurse (RPN) with the British Columbia College of Nurses and Midwives (BCCNM)
  • Bachelor’s Degree in Psychiatric Nursing, with a minimum of 2 years’ experience working with individuals to support their holistic health and wellbeing, especially in the context of trauma, substance use, addictions, and mental illness
  • Broad and comprehensive knowledge of community nursing theory, client & family centered care, substance use and addictions treatment, psychopharmacology and OAT therapy
  • Proof of Covid-19 Vaccination as per the current PHO orders

 

KNOWLEDGE, SKILLS, AND ABILITIES

You understand and acknowledge the systemic racism which continues to plague our health care system, rooted as it is in our shared history of colonialism. You are also deeply aware of the impact of socio-economic health determinants and intergenerational trauma on one’s individual and collective experience of health care. In addition, you have demonstrated ability to provide trauma informed care and counselling services, crisis intervention, and case coordination. You are able to work independently, and in collaboration with others, and possess sound judgment in decision making. You enjoy working together with others, especially in the context of an inter-professional care team. Additionally, you have a strong ethic of client service, and are knowledgeable about the culture and traditions of diverse Indigenous Nations.
