Job Summary:

Reporting to the Home and Community Care Nurse, this position performs home support duties for clients approved for Home and Community Care services and according to the approved Client Care Plan. Duties include client support, house cleaning, client interaction, health observation, and light meal preparation. The Home and Community Care Worker documents daily tasks completed and any observations about changes in the client’s health or home environment. Regular reports and updates are provided to the HCN. Hours of work vary from client to client, and may require travel to various homes. This position requires the individual to complete Occupational First Aid/CPR, Food Safe and WHMIS training. A moderate level of physical fitness is necessary to perform household duties (e.g. standing for extended periods, lifting, kneeling).

 

Duties & Responsibilities:

  • Provide client care as per the approved individual Client Care
  • Maintain a clean, healthy environment for
  • Provide health advocacy for clients and establish links to other health care and social service sectors as
  • Collaborate with the health care team, clients and their caregivers to maximize independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
  • Healthy meal
  • Provide transportation, if insured to do so, to appointments and shopping, etc. OR accompany client to medical appointments, grocery store, pharmacy, as needed.
  • Assist clients and their families in participating in the care
  • Document daily tasks and provide regular updates to HCN.
  • Advise HCN of any work incidents and fill out proper incident reports as
  • Assess and report equipment needs and repairs for
  • Working knowledge of mobility aids
  • Perform delegation of tasks as required and documented by various health professionals, Community Health Nurse, occupational therapists, physiotherapists, physicians,
  • Ensure strict confidentiality when dealing with clients and families of
  • Follow TWN Health Department guidelines, policies and procedures and the TWN Personal Policy and
  • Participate as a member of the TWN Health Department teams.
  • Perform other duties as required.

 

Qualifications:

  • Minimum Grade 12 education plus one year of related experience in house cleaning, maintaining healthy environments, direct home support program delivery,
  • May consider a combination of relevant education and work
  • Knowledge and understanding of the basic concepts of health promotion and
  • Must have Occupational First Aid/CPR – Community Care, Food Safe and WHMIS Certification OR be willing to obtain within a set
  • Must be willing to participate in job-related training or workshops to upgrade skills and knowledge as
  • Clear TB test, to be tested
  • A valid Class 5 BC driver’s license and reliable transportation is
  • Must be willing to obtain business car insurance if transporting
  • Must be able to maintain strict
  • Knowledge of Tsleil-Waututh Nation culture and traditional territory an
  • Previous experience working in a First Nations community and/or organization is preferred.
  • Must successfully pass a Criminal Record Check for the vulnerable
